K MAX+ Debit Card

There are two cases:

1. The cardholder can request to use medical service as a result of accident in the inpatient department (IPD) or outpatient department (ODP) at any hospital in TPA’s and Muang Thai Insurance (MTL)’s list without having to advance medical expenses. However, the amount of such medical expenses must not exceed the coverage limit, and the cardholder must present their K MAX+ debit card and National ID card or passport (in case of a foreign national) to the hospital.
2. For medical treatment as a result of accident at any hospital not in TPA’s and MTL’s list, the cardholder must advance medical expenses. In this case, the cardholder must contact the Call Center of MTL, Tel. 1484, and provide their name/surname, K MAX+ debit card number and contact telephone number. The following documents must be submitted to MTL at 252 Ratchadaphisek Road, Huay Khwang, Bangkok 10310 within 30 days from the date of medical treatment:
   1. Original medical certificate, which clearly specifies the cause of injury
   2. Original receipts
   3. Copy of National ID card and/or other ID cards issued by a government agency
   4. Copy of debit card with the card number corresponding with the name of the cardholder
   5. Copy of the first page of savings deposit/current account passbook linked with the card

Within seven business days after MTL receives the claim form and all required documents.

Medical expenses: 5,000 Baht per accident
Income compensation during hospitalization:
300 Baht / day, up to 30 days per accident

Joint account, account…for…, merchant account with one depositor, account of group of persons, corporate account
